About The Position

Comfort Home Care is seeking a part-time Registered Nurse (RN) for the Worcester area, focusing on psychiatric and home health patients. This role emphasizes work-life balance, allowing nurses to build meaningful relationships with patients and provide compassionate care in their homes. The position offers flexibility, autonomy, and support, enabling nurses to manage their schedules, make clinical decisions, and adapt care plans based on patient needs. Responsibilities include assessing patients, creating and modifying care plans, educating patients and families, coordinating with healthcare providers, and documenting patient visits from home. The goal is to help patients remain independent, avoid hospitalizations, and receive quality care.

Requirements

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• A drive to provide compassionate, patient-centered care.
• Reliable transportation for patient visits.

Responsibilities

• Assess patient's current condition, medical needs, and overall well-being during each visit.
• Create and modify patient care plans based on individual needs and changing conditions.
• Educate patients and families on medication guidance, wound care, and other health-related topics.
• Communicate and coordinate with providers, families, and care teams to ensure timely and appropriate care.
• Document patient visit notes accurately from home for compliance and continuity of care.
