About The Position

The Registered Nurse at the Ft. Lyons Health Center (FLHC) is a key member of the Colorado Coalition for the Homeless (CCH) team, dedicated to delivering high-quality, compassionate healthcare to homeless and at-risk individuals. In this role, you will apply your clinical expertise within an integrated, trauma-informed care model, working closely with a multidisciplinary team to address the complex health needs of our diverse patient population. Your commitment to patient-centered care, coupled with a deep sense of empathy and social responsibility, will drive your efforts to empower patients and improve their overall well-being. This position requires adaptability, resilience, and a proactive approach to overcoming the barriers that often limit access to healthcare for the most vulnerable members of our community. NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Requirements

  • Current licensure in the State of Colorado to practice as a Registered Nurse.
  • Certification in Basic Life Support.
  • A degree in nursing (ADN, BSN or MSN) from an accredited college or university is required.
  • Valid driver’s license required.

Nice To Haves

  • Experience in ambulatory, public, community, mental health, emergency, or critical care nursing is preferred.
  • Pediatric nursing experience is highly desirable.
  • Experience or familiarity with serving diverse or marginalized communities, including those experiencing homelessness, is preferred.

Responsibilities

  • Provide skilled nursing care that meets community standards, ensuring patient care aligns with Colorado Coalition for the Homeless (CCH) protocols and guidelines.
  • Deliver patient-centered care within an integrated, trauma-informed, harm-reduction model, focusing on the physical, emotional, and social needs of each patient.
  • Conduct rapid and accurate triage assessments, both in-person and via telephone, providing appropriate care recommendations based on SSHC nursing protocols.
  • Educate patients about managing chronic and acute health conditions, including diabetes, hypertension, nutrition, wound care, and disease prevention.
  • Safely administer medications, including immunizations and injections, following established protocols and provider orders.
  • Work closely with a multi-disciplinary team, including medical assistants, primary care providers, behavioral health providers, and others, to deliver holistic care.
  • Facilitate patient access to internal programs such as the Health Outreach Program, Housing First, and Medication Assisted Treatment Program.
  • Serve as a member of the Substance Use Disorder (SUD) treatment team, providing case management, crisis intervention, and individual counseling related to medication-assisted treatment.
  • Oversee the care of patients with transitional, chronic, and complex health conditions, ensuring continuity of care and appropriate follow-up.
  • Mentor and guide new nurses and nursing students, providing on-the-job training and support to build their clinical skills and confidence.
  • Accurately document all patient care activities in the electronic health record (EHR), ensuring compliance with legal and organizational standards.
