About The Position
Hospice of Alabama is seeking a Per Diem Hospice RN Case Manager to provide end-of-life care throughout Marshall County. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families through their hospice journey as part of an interdisciplinary care team. Care is grounded in compassion, clinical excellence, and connection, with a focus on patient comfort, quality of life, dignity, and support for loved ones.
Requirements
- Hold a current, unencumbered Registered Nurse license.
- Have at least one year of RN experience in a medical-surgical or acute-care setting.
- Be computer proficient, including typing and use of various programs, with a background in electronic medical records (EMR).
- Maintain current CPR certification if required by the state.
- Have a current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests.
- Partner with the Clinical Director to assess clients’ eligibility and suitability for hospice services.
- Provide direct nursing care and coordinate care to support patients’ comfort and quality of life.
- Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
- Educate and advocate for patients and families, encouraging participation in personalized care plans aligned with their goals.
- Share responsibility for call duties.
