About The Position

DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, and supports patients through assessments, care planning, treatment monitoring, home visits, and on-call assistance. The position works with interdisciplinary teams to promote safe, high-quality outcomes.

The schedule is Monday through Friday with 8-hour shifts; weekends and holidays are off. Scheduling is flexible to support management of the nurse’s caseload. The role is based at 2660 S Broadway Ste A, Rochester, Minnesota.

Requirements

• Active RN license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate Degree in Nursing (ADN) required.
• Basic computer skills, including Microsoft Word and Outlook.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Experience with dialysis, including peritoneal dialysis (PD), home hemodialysis (HHD), chronic, or acute dialysis.
• Experience in patient education or teaching.
• Experience in case management or telehealth.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on peritoneal dialysis procedures and self-care.
• Manage a caseload of peritoneal dialysis patients, including conducting assessments and developing care plans.
• Monitor treatment progress.
• Conduct home visits.
• Provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
