About The Position

The primary responsibility of this Registered Nurse (RN) position is to supervise the day-to-day nursing activities of the facility during their tour of duty. This supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility, and as required by the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) when applicable, to ensure the highest degree of quality care is maintained at all times. The role involves administrative, committee, personnel, nursing care, staff development, safety and sanitation, equipment and supply, care plan and assessment, budget and planning functions, as well as supervisory requirements. The RN is delegated the administrative authority, responsibility, and accountability necessary for carrying out assigned duties.

Requirements

  • Nursing Degree from an accredited college or university.
  • Training in rehabilitative and restorative nursing practices.
  • 2 years or more experience, preferably in a long-term care facility.
  • Ability to read, write, speak, and understand the English language.
  • Ability to read technical procedures.
  • Ability to read and comprehend policy and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to make independent decisions and solve practical problems.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Leadership and supervisory ability and willingness to work harmoniously with and supervise other personnel.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for quality care.
  • Knowledge and skills to provide care appropriate to the age-related needs of the residents served.
  • Be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.

  • Willingness to seek out new methods and principles and incorporate them into existing nursing practices.
  • Ability to relate information concerning a resident’s condition.

  • Current, unencumbered, active license to practice as an RN in this state.
  • Current CPR certification.
  • Must maintain all required continuing education/licensing.
  • Must remain in good standing with the State Board of Nursing at all times.

Responsibilities

  • Supervise the day-to-day nursing activities of the facility.
  • Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities.
  • Participate in developing, maintaining, and updating written policies and procedures for the nursing service department.
  • Ensure that reference materials at the nurses’ stations are current and recommend new materials as needed.
  • Ensure the Nursing Service Procedures Manual is current and that personnel comply with its procedures.
  • Make written and oral reports/recommendations to the Director regarding the nursing service department.
  • Periodically review department policies, procedure manuals, and job descriptions, and recommend revisions.
  • Assist in coordinating nursing services with other resident services for continuity of care.
  • Ensure nursing service personnel comply with their job descriptions.
  • Participate in the facility’s quality assurance program for the nursing service department.
  • Participate in facility surveys conducted by government agencies.
  • Review and update resident discharge plans as required.
  • Assist in planning the nursing services portion of resident discharge plans.
  • Meet with nursing staff and support personnel to plan department services, programs, and activities.
  • Admit, transfer, and discharge residents as required.
  • Complete accident/incident reports.
  • Interpret department policies and procedures to personnel, residents, visitors, and government agencies.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
  • Maintain confidentiality of user ID, password, and resident protected health information.
  • Report suspected or known violations of disclosure policies and unauthorized attempts to access facility information systems.
  • Attend various committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required.
  • Assist in preparing reports for committee meetings.
  • Provide information to the Quality Assurance and Assessment Committee.
  • Participate in developing the agenda for Care Plan Committee meetings.
  • Implement recommendations from committees as instructed by the Director.
  • Ensure sufficient licensed nurses and certified nursing assistants are available for the shift.
  • Develop work assignments and delegate administrative authority to Charge Nurses.
  • Ensure nursing assistants are enrolled in or have graduated from an approved training program.
  • Ensure nurse aide trainees are under direct supervision of a licensed nurse.
  • Make daily rounds to ensure personnel are performing work assignments according to standards and report findings to the Director.
  • Monitor absenteeism and report problem areas to the Director.
  • Ensure departmental policies, including dress codes, are followed.
  • Review and report complaints and grievances from department personnel to the Director.
  • Foster a good working relationship with inter-departmental personnel.
  • Create and maintain a positive and calm environment.
  • Meet with shift personnel to identify and correct problem areas.
  • Review and evaluate the department’s workforce and make recommendations to the Director.
  • Participate in employee performance evaluations, determine staffing requirements, and make recommendations regarding dismissals/transfers.
  • Ensure fair disciplinary action is administered.
  • Report occupational exposures and suspected fraud.
  • Ensure departmental computer workstations are logged off or password protected when unattended.
  • Inform nursing staff of new admissions and room assignments.
  • Ensure rooms are ready for new admissions.
  • Greet and escort new admissions.
  • Make rounds with physicians as necessary.
  • Encourage timely recording and signing of physician progress notes and orders.
  • Inform the Director of untimely physician visits.
  • Consult with residents, families, and physicians on care plans.
  • Notify physicians and next-of-kin of changes in resident condition.
  • Visit residents daily to observe and evaluate their physical and emotional status.
  • Ensure direct nursing care is provided by qualified personnel.
  • Review nurses’ notes for informativeness, descriptiveness, and reflection of resident response to care.
  • Provide direct nursing care as necessary.
  • Start IVs and obtain specimens for lab tests.
  • Take TPRs and blood pressures.
  • Admit, transfer, and discharge residents.
  • Report problem areas and assist in developing corrective action.
  • Keep the Director informed of resident status and related matters.
  • Assist Charge Nurse in monitoring seriously ill residents.
  • Authorize the use of restraints when necessary.
  • Ensure RNs and LPNs comply with medication and supply procedures.
  • Monitor medication passes and treatment schedules.
  • Review medication cards for accuracy and adherence to policies.
  • Inspect treatment areas daily for cleanliness and safety.
  • Ensure residents unable to call for help are checked frequently.
  • Meet with residents and/or family members as necessary.
  • Arrange transportation for discharged residents.
  • Inform family members of resident deaths.
  • Call funeral homes when requested by family.
  • Participate in developing and conducting in-service training classes.
  • Assist in developing annual facility in-service training programs.
  • Assist the Director in planning clinical supervision for nurse aide trainees.
  • Attend and participate in continuing education programs.
  • Assist the Safety Officer in developing safety standards.
  • Identify, evaluate, and classify tasks with potential exposure to blood/body fluids.
  • Ensure personnel participate in training programs for tasks involving potential exposure.
  • Ensure adequate supply of personal protective equipment is available.
  • Ensure personnel follow procedures for use and disposal of personal protective equipment.
  • Monitor personnel to ensure adherence to safety regulations.
  • Ensure nursing service personnel comply with departmental policies and procedures.
  • Ensure work areas are clean and sanitary.
  • Ensure resident care rooms and treatment areas are clean, safe, and sanitary.
  • Ensure personnel follow handwashing and hand hygiene procedures.
  • Ensure personnel participate in fire safety and disaster preparedness drills.
  • Participate in the infection control program.
  • Ensure personnel follow infection control procedures for isolation precautions.
  • Participate in developing procedures for reporting hazardous conditions or equipment.
  • Ensure personnel use safety equipment when lifting or moving residents.
  • Report missing/illegible labels and MSDSs.
  • Recommend department equipment and supply needs to the Director.
  • Ensure adequate stock levels of medications, supplies, and equipment.
  • Participate in developing a preventive maintenance program for equipment and supplies.
  • Participate in developing procedures for the safe operation of nursing service equipment.
  • Ensure only trained personnel operate equipment.
  • Ensure equipment is operated safely.
  • Monitor nursing procedures to ensure efficient use of supplies and avoid waste.
  • Ensure appropriate MSDSs are on file for hazardous chemicals.
  • Participate in developing resident assessments.
  • Participate in developing resident care plans.
  • Encourage resident participation in care plan development and review.
  • Review resident care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure personnel are aware of and refer to resident care plans.
  • Review nurses’ notes to determine if care plans are being followed.
  • Assist in planning, scheduling, and revising MDS, including RAPs and Triggers.
  • Review resident treatments to ensure they align with care plans.
  • Keep abreast of economic conditions and recommend adjustments to nursing services.
  • Report suspected or known incidence of fraud.
  • Assist with the overall supervision and management of the nursing staff.
