About The Position

DaVita is seeking an organized, self-directed Registered Nurse to manage Home Hemodialysis (HHD) patients. The role trains patients and caregivers in a clinic setting so they can perform dialysis independently at home. It combines clinical care, patient education, and case management. The clinic is located in Longview, Texas; home visits are limited.

Requirements

• Current RN license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate Degree in Nursing (ADN).
• Basic computer skills, including MS Word and Outlook.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Dialysis experience, including HHD, PD, chronic, or acute dialysis.
• Patient education or teaching background.
• Case management or telehealth experience.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on Home Hemodialysis procedures and self-care.
• Manage a caseload of Home Hemodialysis patients, including assessments and care planning.
• Monitor treatment progress, conduct home visits, and provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
