About The Position

The Registered Nurse (RN) on the ACT Team is responsible for transcribing medical provider orders, obtaining medical information for clients, and administering medications accurately and on time. This role involves educating clients on medication side effects and assisting with prescription preauthorization. The RN will maintain accurate and timely documentation according to nursing standards, manage a caseload of clients receiving medication services if required, and participate in treatment planning and case reviews. The position requires attendance and participation in various staff meetings and clinical seminars, providing input on client medical care. The RN will also consult with other agencies regarding programs and services, ensure adequate medical supplies and equipment are available, and replenish stock as needed. Responsibilities include administering pregnancy tests and drug screens, monitoring medical equipment according to CLIA standards, and conducting emergency evaluations for high-risk clients, arranging hospitalization when necessary. The RN may also supervise LPN, MA, and CNA team members and manage medical clinics for efficiency and a positive client experience. This role involves occasional exposure to threats of bodily harm and routine exposure to blood, body fluids, or tissue. Reliable transportation and an unexpired driver's license are required for frequent travel between work sites.

Requirements

  • Kentucky Registered Nursing license.
  • One year of Registered Nurse experience.
  • Graduation from an Accredited program.

Responsibilities

  • Transcribes medical provider orders and obtains requested medical information pertaining to clients.
  • Completes medication administration ensuring accurate prescribed dosage and timely delivery, educating clients in management of side effects, and assist preauthorization of prescriptions/medications from pharmacy.
  • Maintains accurate & timely documentation, according to established standards for nursing practice.
  • Manages a caseload of clients receiving only medication services, when required.
  • Maintains treatment planning and case reviews to determine medical treatment, including after-care plans for clients completing treatment.
  • Attends and participates in staff meetings, treatment planning sessions, case conferences, supervisory meetings, and clinical seminars, providing information relating to medical care of the client.
  • Conducts case consultations with other agencies regarding programs and services offered, when requested.
  • Ensures adequate laboratory supplies, medical equipment, and medications are available to meet service demands and replenishes stock on a timely basis.
  • Administers pregnancy tests and drug screens on clients and documents results as needed.
  • Monitors medical equipment and testing per CLIA standards.
  • Conducts emergency evaluations of high-risk clients to determine level of treatment indicated and arranges for hospitalization, as needed.
  • Provides supervision, as requested, to LPN, MA, and CNA team members.
  • Manages the medical clinics to maximize efficiency and create a positive experience for clients.
