About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Registered Nurse works as an RN Case Manager coordinating compassionate, individualized hospice care. The role assesses patients, partners with the interdisciplinary care team, and develops care plans that support each patient’s needs. The position includes on-call responsibilities and services as assigned. The role offers performance recognition and development opportunities. Compensation is based on factors including local labor markets, education, work experience, and certifications. The annual salary range is $71,640 to $107,460 based on full-time employment. Benefits are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.

Requirements

Current RN license specific to the state(s) assigned for work.
At least 1 year of nursing experience.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
Record pain and symptom management changes and outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Regularly communicate with the clinical manager and care team.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Perform other duties as assigned.
