About The Position

This position is for a Registered Nurse in Home Health Case Management, serving the Downers Grove, Lombard, Oak Brook, Elmhurst, and Westmont areas. The role involves developing and maintaining therapeutic relationships with patients and families, providing individualized care, and mobilizing resources to ensure quality, cost-effective care. The nurse will also be involved in patient and staff education, utilizing the nursing process for problem-solving, and demonstrating leadership and collaboration within the healthcare team. This is a full-time position with a schedule of Monday through Friday, 8 am to 5 pm, including rotating weekend work and on-call assignments. A sign-on bonus of up to $3,000 is available for eligible candidates.

Requirements

Nurse, Registered (RN)
A valid driver’s license issued by the Division of Motor Vehicles.
Graduate from an accredited school of nursing
Earn 10 continuing education credits annually
One year of medical surgical experience
Has as demonstrated proficiency for the requirements for RN role
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR

Nice To Haves

Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
