We have a new need for a Registered Nurse - outpatient primary care at the Naval Medical Center in San Diego, CA. This role involves coordinating patient care, ensuring compliance with standards, performing physical exams and health histories, providing health promotions and education, administering medications and interventions, directing and supervising other healthcare professionals, and managing patient care assignments. The nurse will be responsible for recognizing and reacting to emergency situations, communicating effectively with diverse groups, making referrals, conducting research, attending staff meetings, and utilizing electronic health records. Productivity comparable to peers is expected, along with participation in peer review and performance improvement activities. The role requires practicing aseptic techniques, complying with infection control guidelines, and functioning with an awareness of safety procedures. Efficiently managing emergency patient situations, applying awareness of legal issues, and participating in the Family Advocacy Program are also key responsibilities. The nurse must exercise sensitivity to patient rights, maintain accountability for professional practice, and participate in continuing education. Active participation in the command's Performance Improvement Program, including reviewing and evaluating patient care, identifying improvement opportunities, and recommending corrective actions, is essential. The role also includes providing in-service training to staff and assisting with training government healthcare workers. Attending and participating in various meetings as directed is required. Timely, accurate, and concise documentation of patient care is crucial, along with operating automated systems like ADS and MHS Genesis. Participation in clinical staff Performance Improvement (PI) and Risk Management (RM) functions is prescribed. Maintaining a DOD email account and responding to communications are necessary. Documentation of all treatment must be in accordance with clinic directives, and records/reports must be legible, using only approved abbreviations. The nurse will perform patient assessments and triage duties, formulate and execute goal-directed plans of care based on nursing diagnoses and patient outcomes, and evaluate the effectiveness of care. Providing treatment and discharge instructions, performing ongoing assessments of physiological, psychological, and cognitive status, and ensuring a safe and clean environment are also part of the role. Identifying and meeting patient/significant other learning needs, assisting in planning and providing clinical direction to LPNs/LVNs, nursing assistants, and ancillary personnel are required. The nurse will follow up with patients referred by the Primary Care Manager (PCM) or Internal, Behavior Health Consultant (IBHC) to monitor treatment adherence. Participation in PCM or IBHC case staffing and consultations is expected, as is participation in screening processes to improve recognition and intervention for depression and PTSD. The nurse may provide and teach curriculum content for health promotion classes. As part of the MHP team, the employee will raise awareness about behavioral health issues and care facilitator services. Informing the Clinic Manager of administrative/clinical problems and participating in accreditation and Medical Home Port recognition processes are also duties. Engaging other MHP team members to improve documentation and coordinating schedules with team members to ensure adequate access to care are required. Participation in MHP team huddles is also expected.
