About The Position

Amedisys is hiring a full-time Registered Nurse case manager for a Monday–Friday, 8 a.m.–5 p.m. schedule. The nurse helps patients heal at home by assessing their needs, coordinating and delivering care, promoting health and independence, and communicating with the care team. The role includes supervising LPNs and HHAs, providing staff education, completing timely documentation, participating in continuing education, and performing assigned on-call duties. The listed orientation salary is $73,000 to $80,000. Compensation may vary by geographic market, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines when indicated by their needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
