About The Position

This position works as part of a team with the Health Center Manager, medical providers, and clinic support staff to provide quality, coordinated care to patients. The RN will provide clinician support, patient case management, patient education, triage, monitoring of patient flow, and indirect nursing supervision of the medical assistants. Requirements

  • Accredited RN program or higher required.
  • Current unencumbered California Registered Nurse license required.
  • Current BLS/CPR certification required.
  • Strong analytical and problem-solving skills required.
  • Strong interpersonal and customer service skills required; must have ability to work with a diverse population.
  • Strong verbal and written communication skills.
  • Strong computer and data entry skills required.

Nice To Haves

  • Previous RN work experience in a healthcare setting preferred.
  • Bilingual (English/Spanish) language preferred.
  • electronic health record system preferred.

Responsibilities

  • Knowledge of nursing process, scope of practice and standards of care.
  • Utilizes the nursing process (assessment, planning, intervention and evaluation) in patient care and daily activities.
  • Provides and manages patient care under the direction of medical providers, including physical evaluations, assessments, and diagnoses as appropriate.
  • Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within scope of practice.
  • Provides direct one-on-one patient education.
  • Performs independent face-to-face or telephone triage using written triage protocols.
  • Accepts and correctly transcribes verbal or telephone orders from clinicians.
  • Provides follow-up to patients recalled for abnormal labs/tests.
  • Transmits new, updated and refilled prescription orders to pharmacies on behalf of prescribing providers.
  • Performs diagnostic and laboratory tests including but not limited to audiometry, EKG, urine dips, accu-check, urine HCG, hemocue hemoglobin, etc.
  • Performs patient care including but not limited to wound care, birth control visits, blood pressure checks, medication refills, INR/Coumadin services, etc.
  • Processes clinical patient information for providers in patient electronic health records (EHR).
  • Assists with meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
  • Assist with coordinating care to other CommuniCare+OLE services, including dental care, behavioral health, diabetes education, etc.
  • Performs other duties as assigned.
