Requirements

  • Current Registered Nurse (RN) license in the state of practice required
  • Willingness to obtain licensure in additional states as needed
  • Comfortable providing telehealth services
  • Must have a valid driver’s license with an acceptable driving record and current auto insurance
  • Strong clinical judgment and critical thinking skills
  • Ability to work effectively with individuals experiencing behavioral health and substance use challenges
  • Strong communication and interpersonal skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to maintain professionalism in crisis situations
  • Proficiency with electronic health record (EHR) systems
  • Ability to exert up to 10 pounds of force occasionally
  • Ability to perform repetitive hand, finger, and arm movements for typing and documentation
  • Ability to sit for extended periods with occasional walking or standing
  • Ability to meet the physical demands of a sedentary work environment

Nice To Haves

  • For FQHC settings: ACLS certification preferred

Responsibilities

  • Perform comprehensive nursing assessments and evaluate patient health status
  • Complete screenings, document medical histories, and record vital signs
  • Monitor patient health and coordinate care with treatment team members
  • Develop action plans addressing physical and behavioral health needs
  • Provide health education, disease prevention, and risk reduction support
  • Coordinate medical care, appointments, and response to acute health concerns
  • Administer and monitor medications in compliance with regulatory standards
  • Educate patients and families on medication adherence and treatment plans
  • Monitor lab results, medication side effects, and clinical progress
  • Assist physicians and advanced practice providers with exams and procedures
  • Coordinate with pharmacies and assist with medication access programs
  • Maintain accurate and timely documentation in accordance with policies and regulations
  • Respond to medical and behavioral health crises with professionalism
  • Support infection control, hygiene, and overall health monitoring efforts
  • Collaborate with interdisciplinary teams to ensure continuity of care
  • Perform other duties as assigned
