About The Position

The Registered Nurse position at DaVita is primarily located at the DaVita den on-site at Majestic of Southport in Indianapolis, IN, operating Monday through Saturday. This role requires flexibility to support other local DaVita Skilled Nursing Facility (SNF) Dialysis or DaVita In-Center Hemodialysis locations as needed. DaVita SNF Dialysis provides on-site dialysis to patients in a 6-chair den, aiming to reduce patient burden and avoidable healthcare costs. Ideal candidates should have experience in dialysis, be motivated by a start-up environment, committed to improving patient lives, and adept at getting tasks done. Teammates must be comfortable working autonomously and collaborating with cross-functional resources. The role involves comprehensive ongoing patient care, troubleshooting dialysis treatments, coordinating care with SNF staff, and providing patient education. It is a complex yet fulfilling service line focused on achieving and exceeding clinical outcomes, requiring multitasking, autonomy, and critical thinking to assess, diagnose, and implement complete care plans for patients. Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 12 months' of RN experience.
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Basic computer skills and proficiency in MS Word and Outlook.

Nice To Haves

  • Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience.
  • Training/Teaching/Healthcare Education background.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside the SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program.
  • Meet and/or exceed clinical outcomes.
  • Multitask to meet the needs of the service, the patients and our SNF partners.
  • Provide comprehensive patient care in an independent setting.
  • Apply "core" nursing skills to assess, diagnose and implement complete care plans as you look at the whole patient.
  • Commit to improving patient health through clinical goal setting and quality improvement initiatives.
