About The Position

Navitas Healthcare, LLC is seeking a Registered Nurse (RN) – Long-Term Care / Skilled Nursing for an exciting Travel or Local job in Flagstaff, AZ. This role involves providing comprehensive care to residents in a long-term care or skilled nursing facility.

Requirements

  • Active Registered Nurse (RN) license in Arizona.
  • Knowledge of geriatric and long-term care nursing practices.
  • Understanding of aging-related physical, emotional, and psychological changes.
  • Strong clinical assessment and critical thinking skills.
  • Ability to develop and implement patient care plans in a team-based environment.
  • Knowledge of pharmacology and safe medication administration practices.
  • Strong written and verbal communication skills.
  • Ability to observe, assess, and document patient condition and progress accurately.
  • Ability to work collaboratively with interdisciplinary healthcare teams.

Nice To Haves

  • Previous experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings preferred.
  • Experience with MDS assessments and care planning preferred.
  • Experience working with geriatric patient populations preferred.
  • Ability to adapt to varying patient care needs and work schedules.

Responsibilities

  • Administer medications and perform treatments as prescribed by physicians within RN scope of practice.
  • Monitor residents’ physical, emotional, and psychological conditions and report changes promptly.
  • Receive and give shift reports to ensure continuity of care.
  • Communicate effectively with residents, families, and healthcare team members regarding care plans and updates.
  • Perform initial admission assessments for new residents.
  • Assist with Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care planning.
  • Maintain accurate and timely clinical documentation, including assessments, treatments, and progress notes.
  • Provide or supervise assistance with Activities of Daily Living (ADLs), including bathing, dressing, feeding, toileting, hygiene, and mobility.
  • Participate in in-service training, staff education, and development programs.
  • Support quality improvement initiatives using AMS concepts and tools for workflow efficiency.
  • Ensure compliance with all federal, state, and facility regulations regarding nursing practice and medication administration.
  • Contribute to development and implementation of patient care plans within a team nursing model.
