About The Position

Elara Caring is seeking a Registered Nurse primarily focused on Start of Care visits in Mentor, Painesville, Chardon, Ashtabula, Geneva, and surrounding communities in Ohio. The nurse helps patients begin home health care by completing initial assessments, guiding patients and families, and coordinating care. The role also includes ongoing assessments and nursing care as assigned, following the plan of care and physician orders, monitoring patient conditions, maintaining accurate clinical documentation, and supporting safe, high-quality, continuous care.

Requirements

• Current RN license valid in Ohio.
• Graduation from an accredited certificate, diploma, associate, or bachelor’s nursing program.
• At least 1 year of nursing experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home health experience preferred, but not required.

Responsibilities

• Complete comprehensive Start of Care assessments to identify patient needs and establish a foundation for individualized care.
• Complete ongoing assessments and provide nursing care as assigned.
• Provide care according to the plan of care and physician orders.
• Monitor patient conditions, communicate changes, and take appropriate action.
• Help patients and families understand their care, safety needs, and when to seek assistance.
• Coordinate with physicians and the care team to support a smooth transition into home health.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Support safe, high-quality, continuous patient care.
