About The Position

This role is for a Registered Nurse in the Acute Rehab department at Mercy Hospital in Oshkosh, WI. The position is full-time, working day shifts. The nurse will be responsible for driving individualized care plans by monitoring patient progress and adjusting interventions, coordinating multidisciplinary care teams, executing physician and nursing orders, championing smooth care transitions by assessing discharge needs, and advocating for patient wellbeing through ongoing health assessments and clear communication with families.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Responsibilities

Drive individualized care plans by continually monitoring patient progress and adjusting interventions to ensure optimal recovery outcomes.
Coordinate multidisciplinary care teams as the primary point of contact, aligning physicians, specialists, and support staff around a seamless patient experience.
Execute physician and nursing orders with precision, maintaining meticulous communication to protect patient safety and deliver high-quality treatment.
Champion smooth care transitions by assessing discharge needs early and partnering with interdisciplinary teams to prepare patients and families for home.
Advocate for patient wellbeing through active, ongoing health assessments and clear communication with families to keep them informed and supported throughout care.
