About The Position

The Registered Nurse - DP/SNF position is a full-time role requiring the nurse to provide care for patients aged 21 and older. The role involves assessing, treating, and caring for patients, interpreting information based on patient needs, and understanding the growth and development of all patient groups. The nurse must be able to work scheduled hours, including holidays, weekends, and on-call duties, and participate in emergency situations as a designated disaster service worker. The position also requires recognizing and reporting any misconduct or violations of laws, regulations, policies, or standards. The employee must work cooperatively under pressure, meet deadlines, and maintain cooperative behavior with colleagues and supervisors.

Requirements

  • Current licensure as a Registered Nurse in the state of California.
  • Current BLS certification or obtain within orientation process, prior to hands-on patient care.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence if required.
  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Basic computer skills.
  • Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call.
  • Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours.
  • Recognizes affirmative duty and responsibility for reporting perceived misconduct, including actual or potential violations of laws, regulations, policies, procedures, or organization’s standards/code of conduct.

Nice To Haves

  • 3+ years of experience as an RN.
  • Knowledge of long term care regulations.
  • Possesses assessment skills to include respiratory assessment.
  • Possesses ability to initiate, monitor and discontinue all intravenous fluids including total parenteral nutrition and chemotherapy and possesses the knowledge and skills to properly care for central line including porta-caths and triple lumen catheters.

Responsibilities

  • Responsible for the assessment, treatment, or care appropriate to the ages of the patients served (Adult, 21 years and older, or Geriatric).
  • Demonstrates the ability to obtain and interpret information in terms of the patient’s needs, possessing knowledge of growth and development and understanding the range of treatment needed for patient groups.
  • Reviews/revises/records an individualized patient history and assessment, utilizing holistic assessment skills to recognize subtle, covert, or abnormal signs and symptoms, incorporating patient/significant other involvement, discharge needs, psychosocial, biophysical, environmental needs, and teaching needs.
  • Demonstrates problem identification skills based on current physical/behavioral science knowledge base, identifying nursing diagnoses and interdependent nursing problems, and selecting applicable nursing care standards.
  • Develops a care plan that reflects recognition of issues/concerns impacting mutual goals, utilizes interdependent nursing standards, is specific and unique to the individual patient, addresses discharge needs of complex/multi-system patients, and is routinely reviewed/revised based on changing patient status, progress, and discharge readiness.
  • Safely performs nursing care, including safe administration of medication, according to established clinical policy, procedure, and standards.
  • Demonstrates an understanding of the use, actions, and interactions of drugs used in combination for patients with multi-system problems.
  • Troubleshoots equipment and demonstrates skill in clinical problem-solving.
  • Implements interventions when the patient is at covert risk of injury or complication.
  • Evaluates the effectiveness of the plan of care and communicates through report, making recommendations for future care.
  • Evaluates the effectiveness of nursing care by reviewing/revising the plan of care and making recommendations to health care team members.
  • Communicates evaluation of, or changes in, patient status, providing supportive data to the physician or health care team.
  • Evaluates teaching effectiveness based on patient response and revises teaching plan to accommodate patient/family comprehension.
  • Acts as a liaison between the patient/family and members of the health care team.
  • Demonstrates adherence to 'National Patient Safety Goals for Hospitals' as defined by scope of practice.
  • Documentation reflects compliance with established hospital policies and procedures.
  • Demonstrates skill and ability to perform waived testing as per established policy and procedure.
  • Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.
  • Engages the patient through teaching in activities that support the NPSGs.
  • Performs general and unit-specific skills independent of supervision.
  • Adheres to OSHA Guidelines/Universal Precautions as described in hospital policy.
  • Assists in the orientation of new staff.
  • Demonstrates proper infection prevention skills, including standardized protocols or procedures related to central lines and indwelling Foley catheters.
  • Assumes or supports charge nurse role and makes patient care assignments based on patient acuity, staff qualifications, and available resources.
  • Organizes and establishes patient care priorities based on changing patient and/or unit status in a cost-effective manner.
  • Delegates appropriate tasks to unlicensed staff and ensures their completion.
  • Conducts monitors and articulates outcomes to staff and management consistent with the unit's quality assessment plan.
  • Makes recommendations for improvement in patient care.
  • Assists in formulating quarterly reports.
  • Reports quality or risk management issues through appropriate lines of authority.
  • Demonstrates adaptability/flexibility in meeting unit needs, providing staffing support as requested, working extra shifts on short notice, and assisting in unusual clinical situations.
  • Promotes an atmosphere of open communication with respect for the individual.
  • Accepts and responds to changes in a positive fashion.
  • Represents nursing and Sierra View Medical Center in a courteous, helpful, and productive manner.
  • Participates in quality assurance activities and integrates findings into practice.
  • Demonstrates sensitivity and awareness of patient's rights in handling confidential patient information.
  • Represents appropriate practice and directs others to follow or reference hospital policies, procedures, or standards of nursing care.
  • Models appropriate communication and assists other staff in identifying appropriate communication channels and methods.
  • Serves as a clinical resource and provides direction in patient care when working with aides, LVNs, students, and peers.
  • Actively participates in unit problem-solving and communication through involvement in staff/unit rep meetings.
  • Models and articulates the role of professional nursing to other members of the health care team.
  • Maintains current knowledge of health care trends affecting clinical practice.
  • Upholds all aspects of the Nursing Practice Act as defined by the California Board of Registered Nursing.
  • Provides evidence of adherence to all nine ANA Code of Ethics for the nursing profession.
