About The Position

Elara Caring is hiring a Registered Nurse Home Health to provide compassionate care to patients in their homes. The nurse assesses patients’ care needs, provides nursing care in accordance with the plan of care and physician orders, monitors patient conditions, and collaborates with the care team to support safe, high-quality, continuous care. The role also includes educating patients and families, protecting privacy in accordance with HIPAA, and responding appropriately to changes in a patient’s condition. Travel between patient homes and assigned work locations is required.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.

• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
