About The Position

A registered professional nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling. Nurses provide interventions supportive to or restorative of life and well-being, and execute medical regimens prescribed by a physician, dentist, or other licensed healthcare provider.

Requirements

  • License and current registration to practice as a registered professional nurse in New York State.
  • Knowledge of professional nursing concepts, principles, and practices.
  • Knowledge of concepts and principles from the biological sciences that are supportive of nursing practice.
  • Knowledge of concepts and principles of health promotion and disease prevention.
  • Knowledge of abnormal signs and symptoms relating to physical and mental illness, as well as indicators of wellness.
  • Knowledge of medications and drugs, common dosages, their psychological, physical, and physiological effects, and possible adverse reactions.
  • Knowledge of emergency treatments and interventions.
  • Knowledge of teaching and consulting techniques.
  • Knowledge of the Agency's routines, procedures, and policies.
  • Knowledge of cultural, ethnic, and linguistic needs of clients relating to cultural competence.
  • Ability to recognize changes in signs and symptoms that warrant the immediate action of a physician.
  • Ability to develop, evaluate, implement, and modify a plan of nursing intervention to meet the needs of individual clients.
  • Ability to secure the cooperation of and work effectively with others.
  • Ability to prepare accurate and timely reports.
  • Demonstrate the ability to use Microsoft Office applications including, but not limited to Outlook, Excel and EPIC care link.

Nice To Haves

  • At least one year post RN licensure clinical nursing experience working with individuals with mental disabilities.

Responsibilities

  • Responsible for the overall safety, health, and welfare of all residents served.
  • Documents appropriately in H&H electronic medical records (EPIC) on residents served, including health assessment, goal planning, health notes, and any changes or recommendations in resident health status and warm handoff notes for patients moving to another program.
  • Completes discharge notes in EPIC for residents leaving the program.
  • Completes the DHS Institutional Referral Form and Reasonable Accommodation Form if needed.
  • Communicates and documents feedback from outside affiliates on incoming and current patients.
  • Attends weekly meetings with ICL and H&H.
  • Must be available via phone for staff when not in office.
  • Documents and assesses vital signs parameters of high-risk patients weekly or as needed and recognizes when to call 911.
  • Must be able to triage to appropriate service and level of care between onsite care and telehealth services.
  • Provides ongoing medication monitoring training and refresher for direct support staff.
  • Provides feedback on medical assessment to determine appropriateness for admission and discharge from the medical respite.
  • Responsible for medication cart review.
  • Provides feedback on review of the medication administration record (MAR).
  • Ensures that the control substance count sheet is correct and confirms that there is a signature on the count verification sheet for each shift.
  • Provides patient education to residents regarding their treatments, medications, and dietary needs to improve their health outcomes.
  • Ensures that clinical policies, procedures, and standards of care are being followed.
  • Assists in completing consultation forms for residents with medical/psychiatric and specialty appointments.
  • Must be able to effectively respond to off-hours resident needs that may arise at the Medical Respite.
  • Assists in performing minor first aid and wound care if needed.
  • Responsible for infection control / bloodborne pathogens standards and compliance using universal and standard precautions.
  • Reviews medication refrigeration/freezer log.
  • Serves as a liaison with pharmacies as well as all consulting healthcare practitioners and medical offices. Ensures start of care plan is in place for new clients if needed.
  • Provides weekly clinical notes to the team leader if needed.
  • Attends weekly/monthly housing, assessment, staff meetings, and supervision.
  • Anticipates, analyzes, diagnoses, and resolves problems.
  • Demonstrates initiative to learn and implement new concepts, technologies, and/or methods.
  • Demonstrates critical thinking, assessment, and problem-solving skills.
  • Demonstrates excellent interpersonal skills.
  • Works collaboratively and productively with others.
  • Demonstrates the ability to use Microsoft Office applications including, but not limited to Outlook, Excel, and EPIC care link.
  • Performs other incidental and related duties as required and assigned.
  • Provides crisis intervention.
  • Gives lectures and participates in community activities as a medical and mental health advocate.
  • Provides information and conducts lectures and discussions on various health topics, such as: the effect and side effects of medications, well-being, coping skills, dealing with anxiety, anger, etc.
