About The Position
This role offers a rare Monday–Friday, 8am–5pm schedule in home health nursing, providing stability and the chance to deliver skilled, one-on-one nursing care in patients' homes. You will manage a full scope of skilled nursing work including wound care, medication management, IV therapy, and symptom management, paired with hands-on patient and caregiver education. You'll also lead care coordination, supervising LPNs/LVNs, home health aides, and other support staff as part of an interdisciplinary team.
Requirements
- Graduate of an approved nursing program with current RN licensure in the state
- At least 1 year of experience as an RN
- Current CPR certification, maintained throughout employment
- Valid driver's license with a properly insured, reliable vehicle for travel between patient locations
Responsibilities
- Conduct comprehensive nursing assessments covering physical, emotional, and psychosocial needs
- Build and adjust individualized care plans alongside your interdisciplinary team
- Deliver direct skilled care, including wound care, IV therapy, and pain/symptom management
- Educate and support patients and families through each stage of care
- Supervise LPNs/LVNs and home health aides
- Participate in on-call rotations, admissions, or urgent response as needed
- Manage visit scheduling and documentation to stay compliant with regulations
- Monitor patient progress and adapt care plans as conditions change
