About The Position

Amedisys is hiring a registered nurse case manager to provide home health care. The role supports patients in healing at home by assessing their needs, collaborating with the care team, developing and implementing plans of care, and promoting health, independence, and self-care. The nurse also supervises LPNs and home health aides, documents care, communicates patient progress, provides staff education, participates in continuing education, and performs on-call duties as assigned. The listed pay range is $46.96–$58.70. Compensation may vary by location, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP of Clinical.
• Current RN license for the state or states assigned.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on patients’ identified needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
