The Registered Nurse oversees and coordinates patient care delivered in the home environment. This role blends clinical nursing expertise with care coordination, patient advocacy, and ongoing assessment to ensure patients receive safe, effective, and person-centered care at home. The Registered Nurse works closely with caregivers, physicians, therapists, and community partners to support optimal health outcomes and reduce avoidable hospitalizations. The Registered Nurse documents nursing actions of all care given to individuals and informs physicians and family accordingly. Conducts nursing assessment visits as needed. Follows policies and procedures according to operation, state, and regulatory standards. Completes supervisory visits based on compliance and regulations to include review of the plan of care, caregiver competency assessment, and completion of required documentation. Implements nursing support for individuals in a home setting. Works with the Director of Nursing/Nurse Supervisor and/or Branch Manager to address the health needs of individuals. Participates in staff meetings, in-services, and other meetings as needed. Ensures compliance with physician’s verbal orders and medical reports.
