About The Position

The experienced Registered Nurse is responsible for assisting providers with the evaluation and management of all patients presenting to the Health Center and is a critical partner in our Health Centers. This role involves conducting patient assessments, obtaining patient history, assisting in the development of care plans and coordination of care, and supporting clinicians in managing complex patient healthcare needs. The nurse will also lead and support back and front office staff regarding patient concerns or center needs, implement provider orders, manage medication refills, provide patient/family education, identify opportunities for preventative screening, and notify members of test results. Additionally, the role includes triaging patient calls, coordinating with the front office team for appointments, communicating with providers and other departments, setting up patients for procedures and exams, obtaining vital signs, utilizing phlebotomy skills for blood draws, ensuring day-to-day clinic operations, managing supplies, and documenting all care provided. Additional responsibilities aligned with certification or licensure may be assigned.

Requirements

  • Graduate of approved RN program
  • Current licensure as a Registered Nurse (RN) in Georgia and the ability to maintain without restrictions.
  • At least one year or more years of experience in a primary care clinic or provider office.
  • Current BCLS is required
  • Phlebotomy skills
  • Solid Computer skills including Electronic Health Records
  • Excellent customer service skills required

  • Ability to work health center hours (schedule will be 4 – 10 hour days within the hours of: Monday - Friday 6am – 6 pm

Responsibilities

  • Conduct patient assessment; obtain patient history
  • Assist in development of care plans and coordination of care.
  • Support the clinician to help manage the complex healthcare needs of patients, lead and support the back and front office staff with regards to patient’s concerns or center needs
  • Assist in implementing provider orders and order follow-up, medication refills, patient/family education, identifying opportunities for preventative screening, notification of member’s test results.
  • Triage patient calls and concerns
  • Coordinates with front office team to schedule appointments and meetings.
  • Communicate effectively to provider and other departments to resolve issues.
  • Set up all patients for procedures, exams and obtains vital signs.
  • Utilize phlebotomy skills for Blood draws
  • Ensure day-to-day operations in the clinic, use your organizational skills to help manage the health care needs of our varied patient population.
  • Properly document your efforts of all the care and information provided to our patients.
  • Maintain supplies in exam room
  • Additional responsibilities in alignment with the scope of your certification or licensure
