About The Position

Valley Regional Home Health is seeking a Registered Nurse for a part-time or full-time position, with a requirement to work Saturdays and Sundays after an initial training period. The training period involves Monday through Friday work for at least one month, typically from 8:00 am to 4:30 pm. Once independent, RNs can adjust their schedules to accommodate patient care in the home environment. RNs will manage 3-5 patient visits daily and will be required to drive between locations. The agency, operating since 2013, has a strong nursing team and a significant patient census, aiming to serve the community with a focus on employee growth and a positive professional culture.

Requirements

Registered nurse with current licensure to practice professional nursing in the state.
Graduate of National League for Nursing accredited school of nursing.
Maintains a current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Willing to drive around the job posting area.
Demonstrates excellent observation, problem solving, verbal and written communication skills.
Ability to work well with others and communicate time-sensitive information effectively.
Shows ability to organize and prioritize workload independently.
Willing to train Monday through Friday for at least one month.

Nice To Haves

One year of home health experience preferred, but not required.

Responsibilities

Completes an initial assessment of patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es).
Reads documentation on patient status and physician orders.
Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs, provides education on plan of care.
Provides health care instructions to the patient as appropriate per assessment and plan of care.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
