About The Position

Valley Regional Home Health is seeking a Registered Nurse for a part-time or full-time position, with a requirement to work Saturdays and Sundays after an initial training period. The training period involves Monday through Friday work for at least one month, typically from 8:00 am to 4:30 pm. Once independent, RNs can adjust their schedules to accommodate patient care in the home environment. RNs will manage 3-5 patient visits daily and will be required to drive between locations. The agency, operating since 2013, has a strong nursing team and a significant patient census, aiming to serve the community with a focus on employee growth and a positive professional culture.

Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Willing to drive around the job posting area.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills.
  • Ability to work well with others and communicate time-sensitive information effectively.
  • Shows ability to organize and prioritize workload independently.
  • Willing to train Monday through Friday for at least one month.

Nice To Haves

  • One year of home health experience preferred, but not required.

Responsibilities

  • Completes an initial assessment of patient and family to determine home care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Reads documentation on patient status and physician orders.
  • Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs, provides education on plan of care.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
