Practices in accordance with the standards of professional practice and code of ethics for registered nurses as outlined by the British Columbia College of Nurses and Midwives (BCCNM), as well as within a client and family centred care model and the vision and values of the organization. Provides care and services to new and follow up clients in the Adult Metabolic Disease Clinic. Performs preliminary assessments, collaborates with appropriate health care professionals, identifies client needs, and implements strategies and treatments through ongoing care and monitoring. Provides client/family education around disease and medication management, including information for actions needed if complications develop. Acts as a resource to clients and families, coordinating client access and involvement in relevant programs/services within their community. Plans, implements, reviews and revises client education sessions and materials as needed. Maintains accurate and complete databases and tracking systems for clients on special therapies.
