About The Position

Transitions Hospice is seeking a Registered Nurse to provide hospice care in Oak Park, Cicero, Chicago’s North Side, and surrounding areas. The role centers on protecting patients’ dignity and comfort while providing physical, emotional, and spiritual support in their homes. The RN serves as a case manager and clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. The schedule is 8 AM–8 PM, alternating between four-day and three-day weeks, with weekends in rotation. The role includes on-call rotation and rotating holiday requirements. The pay range is $85,000–$95,000 per year.

Requirements

Active RN license in the state of employment.
Graduate of an accredited school of Nursing.
Compassion for patients and families navigating end-of-life care.
Strong clinical judgment and ability to work independently in the field.
Excellent communication skills across care teams and families.

Nice To Haves

One or more years of experience in hospice, home health, or a related organization is preferred.

Responsibilities

Conduct comprehensive, ongoing assessments to understand each patient’s evolving needs.
Build and adjust personalized care plans in collaboration with physicians and care teams.
Administer medications and treatments according to physician orders.
Maintain accurate, timely, and meaningful clinical documentation.
Serve as a communication bridge between families, physicians, and community health partners.
Educate patients and caregivers so they feel confident and supported.
Participate in the on-call rotation according to team policy.
Coordinate care and guide the interdisciplinary team.
