About The Position

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate care to patients in their homes. The nurse assesses patients’ care needs, provides nursing care according to the plan of care and physician orders, monitors patient conditions, and supports safe, continuous care. The role also includes educating patients and families and protecting patient and staff privacy in accordance with HIPAA.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
