About The Position
This is a community nursing role for a Registered Nurse (RN) involving scheduled visits to care for patients in various settings. The role requires working with patients of all ages with differing complexity of care needs, playing a key role in helping them remain comfortable in their homes for as long as possible. Requirements
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver’s license and access to a vehicle is required
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs

Responsibilities
- Using appropriate assessment tools/techniques to assess patients’ individualized needs
- Executing and continuously monitor care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and update professional patient records related to patient encounters and plans
