About The Position

Mission Healthcare is seeking a Home Health Registered Nurse to provide patient-centered care in the Portland, Oregon territory. The role involves assessing patients and families, planning and coordinating home care, and counseling patients and families about nursing and related health needs. The position is available part time on Saturday and Sunday, or full time on a Saturday–Wednesday or Wednesday–Sunday schedule. The listed pay range is $55–$60 per hour, depending on experience.

Requirements

• Current and valid state RN license.
• At least 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) with hands-on CPR certification.
• Valid driver’s license and auto insurance.

Nice To Haves

• Associate’s or bachelor’s degree in Nursing.
• 1 year of home health experience.

Responsibilities

• Conduct thorough assessments of patients and their families to determine home care needs.
• Provide comprehensive physical assessments and document patients’ illness histories.
• Develop and update individualized care plans with active participation from patients and their families.
• Identify discharge-planning needs and ensure they are addressed before patient discharge.
• Counsel patients and families on nursing and related health needs.
