About The Position

Find your purpose by treating patients where they’re most comfortable – in their home. As a Home Health Team Registered Nurse, you will work independently providing skilled care for 5 to 6 patients per day. Common duties include wound management, infusion therapy, assessments, education and training for patients on their new diagnosis, and more. We provide extensive training and ongoing RN office support while you’re making home visits to patients. This position will primarily work in Buffalo, Monticello, Annandale, Maple Lake, Clearwater and surrounding areas.

Requirements

• Graduate from an accredited school of nursing.
• Current Minnesota Registered Nurse license.
• Current American Heart Association (AHA) BLS Healthcare Provider Card.
• Minimum of one year of RN experience or 2 years experience in a Home Health setting.
• Reliable transportation.
• Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage.

Responsibilities

• Provide skilled care for 5 to 6 patients per day.
• Perform wound management.
• Administer infusion therapy.
• Conduct patient assessments.
• Educate and train patients on their new diagnosis.
