About The Position

Amedisys is hiring a Registered Nurse Case Manager for home health care. This full-time, day-shift role follows a weekend schedule of Friday through Sunday or Saturday through Monday. The nurse provides patient assessments and care planning, supports patients in healing and achieving independence at home, supervises LPNs and home health aides, and coordinates with the care team. The position includes assigned on-call responsibilities and participation in clinical development and continuing education.

The advertised pay range is $70,000–$85,000. Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP of Clinical.
• Current RN license for the state or states in which the employee is assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines based on patients’ identified needs or documents the rationale for not making referrals.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
• Supervises LPNs and home health aides.
• Completes documentation accurately and on time.
• Communicates patient progress regularly to the clinical manager and care team.
• Plans and provides staff education.
• Performs assigned on-call responsibilities and provides on-call services to patients and families.
• Participates in clinical development and continuing education programs.
• Performs other duties as assigned.
