About The Position

Beacon Home Health is seeking a Registered Nurse to provide and direct nursing care for patients in their homes in the Goodlettsville, Tennessee area and surrounding communities. The RN works under physician-prescribed care plans and follows applicable laws, regulations, agency policies, and state-specific nursing practice guidelines. The role focuses on promoting wellness, preventing re-hospitalization, and coordinating continuity of care among physicians, agency personnel, patients, families, and community resources. The position is listed as PRN.

Requirements

• At least one year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute daily to multiple patient residences within the assigned geographic area.

Responsibilities

• Create, implement, execute, and coordinate plans of care to promote wellness and help prevent patient re-hospitalization.
• Coordinate and exchange care information with physicians, agency personnel, patients, families, and community resources.
• Conduct initial and/or comprehensive nursing evaluation visits, determine the primary focus of care, and develop plans of care within state-specific guidelines.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes describing patient condition, goals, interventions, and response to care.
• Establish and maintain an effective and efficient visit route.
• Provide clinical services within the scope of practice, according to the plan of care, from admission through discharge.
