About The Position
Remedy Home Health Care is seeking experienced and compassionate Registered Nurses (RNs) to join our growing team. Requirements
- Active California Registered Nurse (RN) license (required)
- Minimum 1 year of RN experience (home health experience preferred)
- Strong clinical, communication, and organizational skills
- Valid driver’s license and reliable transportation
- Ability to travel within Los Angeles County
Nice To Haves
- Experience in home health, rehab, geriatric care, or community care is a plus
- Prior RN Home Health experience
- Public Health Nurse (PHN) certification
- Experience with mobile nursing or home-based care models
Responsibilities
- Provide skilled home health nursing care, including assessments, wound care, medication management, and treatments
- Develop and implement individualized home health care plans
- Monitor patient progress and adjust care plans accordingly
- Educate patients and families on disease management, rehab, and senior care
- Coordinate care with physicians, therapists, and interdisciplinary teams
- Complete accurate and timely documentation in compliance with home health regulations
