About The Position

Baystate Health is seeking an experienced Primary Care Nurse to join Greenfield Family Medicine in Greenfield, MA. This is a full-time position (40 hours per week) and is eligible for a sign-on bonus. The role involves providing independent quality triage and patient care, education and support for patients with chronic substance use health conditions, and developing care plans in partnership with patients and medical staff. The nurse will also triage and coordinate patient care, promote health and wellness, and advocate for patient rights and beliefs.

Requirements

• Massachusetts license as an RN
• BLS CPR certified
• Minimum of 2 years of nursing experience
• Experience providing clinical care, education, and treatment to patients with substance abuse diagnoses or primary care experience
• Demonstrated interest to work with patients from diverse economic and cultural backgrounds
• Excellent verbal and written communication and interpersonal skills.
• Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff
• Diploma in Nursing (Required)
• Basic Life Support - American Heart Association
• Registered Nurse - State of Massachusetts

Nice To Haves

• 1 year of outpatient experience (for sign on bonus eligibility)

Responsibilities

• Provides independent quality triage and patient care utilizing strong assessment skills, point of care testing and quality assurance checks
• Provides education, ongoing assessment and support to patients with chronic substance use health conditions
• In partnership with patient and medical staff, develop and implement plan of care for complex patients including medical and/or psychosocial disorders.
• Triages and coordinates patient care ensuring access to care and patient success.
• Promotes health and wellness for individuals and groups of patients.
• Demonstrates respect and advocates for the patient’s rights, values and belief systems.
