About The Position
The Leo A. Hoffmann Center (LAHC) is a nonprofit Psychiatric Residential Treatment Facility (PRTF) serving adolescent males ages 11-17 with complex mental health and behavioral health needs. A PRTF provides a highly specialized level of care that is more intensive than traditional residential or day treatment services, while offering a more therapeutic and relationship-based environment than an inpatient psychiatric hospital. At LAHC, every client receives individualized treatment designed to address their unique mental, emotional, medical, educational, and social needs. Requirements
- Must be 21 years of age or older.
- Current Minnesota Registered Nurse (RN) license in good standing.
- Strong communication, assessment, and critical-thinking skills.
- Ability to effectively relate to and interact with clients, co-workers, families, providers, and members of the public in a professional and positive manner.
- Commitment to providing compassionate, client-centered care.
Nice To Haves
- Preferred specialized training or at least two years of experience working with individuals experiencing mental health challenges.
Responsibilities
- Assess and monitor the physical and mental health needs of clients.
- Coordinate ongoing medical services and healthcare appointments.
- Administer, monitor, and document medications in accordance with physician orders and facility policies.
- Collaborate with the agency psychiatrist and treatment team to support client psychiatric care.
- Communicate medical concerns, observations, and recommendations to the interdisciplinary treatment team.
- Provide accurate, timely, and thorough documentation consistent with client treatment plans and regulatory requirements.
- Assist in maintaining compliance with state licensing standards and healthcare regulations.
- Support a safe, therapeutic, and trauma-informed environment for clients.
