About The Position

Nightingale Visiting Nurses of Connecticut is seeking a Registered Nurse to provide one-on-one home health care in patients’ homes in the Stamford and Norwalk, Connecticut areas. The role offers autonomy, flexible scheduling, clinical support, and per-visit compensation starting at $55 per visit, with rates based on experience. The nurse delivers compassionate care to help patients remain safe and independent at home.

Requirements

• Current Registered Nurse (RN) license in Connecticut.
• Physical exam within the last 12 months.
• TB test within 12 months and current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to attend a 1–3 day orientation at the Stamford office.

Nice To Haves

• OASIS and WellSky (Kinnser) experience preferred.
• Medical-surgical and wound care experience preferred.

Responsibilities

• Conduct patient admissions and develop individualized Plans of Care.
• Obtain medical histories and perform physical assessments.
• Monitor patient status, vital signs, and clinical progress in the home setting.
• Deliver skilled nursing care in compliance with HIPAA and agency standards.
• Evaluate patient outcomes and adjust care plans as needed.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Coordinate services with LPNs, HHAs, and therapy providers.
• Complete accurate documentation and submit clinical notes in a timely manner.
