About The Position

PruittHealth @ Home is hiring compassionate Registered Nurses for home health care in York County. The role offers one-on-one patient care, autonomy, flexible scheduling, and clinical support. Home Health RNs provide skilled nursing services in patients’ homes, manage assessments and care plans, coordinate with an interdisciplinary team, and support continuity, quality, and safety of care. The position is offered as full-time and PRN, with a pay range of $74,880–$90,480 based on experience.

Requirements

• Active, unrestricted Registered Nurse (RN) license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• At least 1 year of RN experience.

Nice To Haves

• Home health or acute care experience.
• Experience with aging or adult patient populations.
• Strong organization and time-management skills, including the ability to multitask and prioritize a busy caseload.
• Confidence working with technology and electronic documentation systems.
• OASIS experience.

Responsibilities

• Provide skilled nursing visits in patients’ homes.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist.
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.
