This role involves assisting in the development of care protocols, key measures, and standards of care. The nurse will contribute to establishing program goals and objectives for patient care and management. A key aspect of the role is engaging patients and their families in developing healthcare strategies to improve quality of life through education, empowerment, and access to appropriate health services. The nurse will monitor trends and implement strategies to help patients access the appropriate level of care. This position also involves participating in team-directed improvements using and teaching the PDSA change model, and engaging in daily huddles. The role includes developing and conducting staff training on new equipment, policies, procedures, and electronic systems. The nurse will monitor patient electronic medical records, produce reports, and follow data entry requirements. They will track and report key measures for disease management, referrals, and preventive care. The role requires attending meetings to achieve program goals and actively participating in performance improvement activities and committees. Responsibilities also include managing triage for walk-in patients, responding to patient phone calls, and performing after-hours on-call duties. Patient education on health promotion, lifestyle modification, disease prevention, self-care, and medication usage is crucial. The nurse will explain potential diagnoses to patients, coordinate patient flow, and provide nursing care according to the Kentucky Registered Nurse Scope of Practice. Performing prenatal workups may be required. Adherence to the corporate compliance program and HIPAA regulations is mandatory. Other duties as assigned.
