Registered Nurses provide medical and psychiatric support in a person's community, meeting clients where they are at on their journey to recovery. This role offers work/life balance while serving clients experiencing mental illness who often lack access to adequate healthcare. Through care planning, assessment, and implementation, the nurse will be an integral part of a client's growth and recovery, working closely with clinical teams including consulting psychiatrists and medical doctors. Responsibilities include providing health education and training, removing barriers to healthcare through home visits, developing treatment plans, administering medication, scheduling appointments, coordinating care, attending to medically related emergencies or crises, and logging medical care. The culture is collaborative and supportive, offering tools and resources for client care, with opportunities for training and professional development in areas like harm reduction and trauma-informed care. The Homeless Outreach Program (HOP) provides assertive outreach, linkage to primary and psychiatric care, and resource referral to people experiencing homelessness with serious mental illnesses, aiming to get them into stable housing and help them recover.
