About The Position

Join our Hospice House/Inpatient Unit team as a Registered Nurse and provide compassionate, patient-centered care to individuals with life-limiting illnesses. You’ll support patients and their families throughout their hospice journey while working closely with an interdisciplinary team in a collaborative environment focused on comfort, dignity, and clinical excellence.

Requirements

  • Current Registered Nurse (RN) license in good standing with no disciplinary actions.
  • Current CPR certification.
  • Strong clinical assessment, symptom management, and care planning skills.
  • Experience documenting patient care in an electronic medical record system.
  • Valid Ohio driver’s license, reliable transportation, and proof of auto insurance.
  • Ability to successfully complete a criminal background check and 10-panel drug screening.

Nice To Haves

  • Certified Hospice and Palliative Nurse (CHPN) certification preferred; support is available for eligible employees pursuing certification.

Responsibilities

  • Provide skilled nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
  • Assess and address patients’ physical, emotional, psychosocial, and spiritual needs.
  • Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and the interdisciplinary care team.
  • Monitor changes in patient condition and communicate significant updates to physicians to ensure appropriate levels of care.
  • Evaluate and adjust nursing interventions, medications, and care needs as patient conditions change.
  • Participate in interdisciplinary team meetings, care conferences, and patient admissions.
  • Maintain accurate and timely documentation of assessments, interventions, physician orders, and patient status in the electronic medical record.
  • Provide thorough patient handoffs using effective communication methods, including SBAR, to promote continuity of care.
  • Provide guidance and oversight to personal care specialists, volunteers, and other caregivers to ensure care is consistent with the established plan.
  • Collaborate with other departments and team members to support exceptional patient care.
  • Participate in quality improvement initiatives designed to enhance patient outcomes and organizational excellence.
  • Serve as a preceptor for new team members when assigned.
