About The Position
Visiting Nurse Association of Florida is seeking a Registered Nurse to provide home health visits in the Boynton Beach, Delray Beach, Boca Raton, and South West Palm Beach areas. The role involves delivering skilled nursing care in patients’ homes and supporting their recovery and independence. Requirements
- Active Florida RN license.
- Minimum one year of nursing experience.
- Strong assessment and critical thinking skills.
- Ability to work independently.
Nice To Haves
- Home health experience preferred.
- OASIS experience preferred.
Responsibilities
- Perform skilled nursing visits in patient homes.
- Complete OASIS assessments and documentation.
- Develop and manage individualized care plans.
- Educate patients and caregivers on disease management and safety.
- Coordinate care with physicians and interdisciplinary team members.
- Monitor patient progress and communicate changes in condition.
