About The Position
We’re seeking five full-time Registered Nurses (RNs) to join our Dublin, Ohio team! As a Home Health RN, you’ll provide one-on-one skilled nursing care in patients’ homes, coordinate with physicians and care teams, and ensure top-quality outcomes for every patient. Requirements
- Active RN license in Ohio
- 2+ years of home health nursing experience
- Skilled in managing diverse home health diagnoses
- Strong clinical assessment and critical thinking skills
- Valid driver’s license and reliable transportation
Nice To Haves
- BSN preferred
Responsibilities
- Conduct initial and ongoing assessments — including Start of Care (SOC), Recertifications, Resumptions, and Discharges
- Develop, implement, and evaluate individualized plans of care
- Administer medications, wound care, and treatments
- Educate patients and families on care routines, medications, and disease management
- Collaborate closely with physicians, therapists, and the interdisciplinary team
- Document care accurately in WellSky EMR (or similar)
- Ensure compliance with Medicare, Medicaid, and agency standards
- Participate in weekend rotation as needed
