About The Position

The Registered Nurse - MD Primary Care Program at Luminis Health is responsible for efficiently and effectively providing patient triage and a variety of nursing treatment functions. This includes assessment of symptoms, prioritizing patient appointments, determining patient disposition, and providing feedback to the provider. The role involves professional and tactful communication with patients regarding medical questions and medication inquiries, handling follow-up calls, and assessing symptoms using approved guidelines to determine the appropriate level of care or self-care advice. The nurse will also provide patient education, advise on health maintenance and disease prevention, monitor the provider's in-basket, debrief with the provider as needed, and document all communication in EPIC. Additionally, the role involves communicating with insurance companies for authorizations, maintaining accurate reports, participating in quality improvement activities, and collaborating with other healthcare professionals to support patient needs.

Requirements

  • Graduate of an accredited School of Nursing.
  • Two years of related clinical nursing experience.
  • Current Maryland Board of Nursing license.
  • CPR through the American Heart Association (BLS for Healthcare Providers level).

Responsibilities

  • Assure that communication with each caller will be handled in a professional and tactful manner.
  • Communicate with patients regarding their medical questions, medication inquiries, etc.
  • Handle follow-up calls to patients per provider request.
  • Triage incoming patient calls regarding medical concerns and determine appropriate disposition (schedule appointment, Urgent Care, ER).
  • Assess and triage caller’s symptoms using the nursing process and approved guidelines to assist in obtaining the appropriate level of care and/or self-care advice.
  • Provide patient education in alignment with treatment goal(s).
  • Advise patients on health maintenance and disease prevention.
  • Monitor provider in basket to assist the provider in patient intervention and/or forwarding message to the provider for additional clinical guidance.
  • Debrief/communicate with provider as needed.
  • Document all patient-provider-staff communication in EPIC.
  • Communicate with insurance companies to obtain required authorizations, if applicable.
  • Maintain accurate, detailed reports and records.
  • Participate and oversee performance and quality improvement activities.
  • Work collaboratively with RN Care Managers, LPNs and Medical Assistants in support of clinical providers and patients’ needs.
