About The Position

BAYADA Home Health Care is seeking an experienced Registered Nurse for a per diem opportunity performing weekend Start of Care home health visits in Montgomery County, Maryland. Home health experience and Medicare OASIS experience are required. The nurse is an integral member of a multidisciplinary health care team, providing rehabilitative care and skilled nursing to clients so they can receive the medical care they need while remaining at home.

Requirements

• Hold a current Registered Nurse license in Maryland.
• Have at least one year of recent, verifiable clinical (medical/surgical) experience as a licensed Registered Nurse.
• Have home health experience and Medicare OASIS experience.
• Graduate from an accredited and approved nursing program, as indicated by a school transcript or diploma.
• Be able to work independently and manage time effectively.
• Have strong interpersonal skills.
• Be able to travel to assigned cases.

Nice To Haves

• Prior home care experience is strongly preferred.
• Prior experience with electronic medical records (EMR) is preferred.
• Solid computer skills.

Responsibilities

• Follow a designated care plan in accordance with the patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Provide case management and coordination.
• Accurately document observations, interventions, and evaluations related to client care management and services provided using a touch pad tablet.
