Registered Nurse - Weekend SOC - Senior Living Visits
About The Position
BAYADA Home Health Care is seeking a Registered Nurse (RN) for a Per Diem opportunity to service clients within senior living communities in Baltimore County, Carroll County, and Harford County, MD. This role focuses on admissions and requires availability on Fridays, Saturdays, and Sundays, or Saturdays and Sundays between 8:30 am and 5:00 pm. As an RN, you will be a key part of a multidisciplinary team providing rehabilitative care and skilled nursing to clients, enabling them to receive medical care at home. This position offers the benefit of home health care without the typical home health commute.
Requirements
- A current license as a Registered Nurse in Maryland.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Current Home Health and Oasis experience are required.
- One year prior clinical experience as a licensed Registered Nurse is required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
- Must be available Friday, Saturday and Sunday or Saturday and Sunday between 8:30am-5:00pm.
Nice To Haves
- Prior home care experience strongly preferred, but not required.
Responsibilities
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
