Registered Nurse (RN)- Sacramento/Placerville (Salaried)
About The Position
Aspire Home Healthcare is seeking a compassionate and skilled Registered Nurse (RN) to join their Home Health team serving Sacramento, Folsom, and Placerville. This full-time salaried position offers consistent compensation, meaningful patient relationships, and the opportunity to provide personalized care while working independently in the field. The Registered Nurse (RN) is responsible for providing direct patient care and case management in accordance with physician orders and agency standards. Responsibilities include completing patient admissions, developing individualized care plans, coordinating interdisciplinary services, and supporting positive patient outcomes in the home setting.
Requirements
- Active California Registered Nurse (RN) license
- Current physical examination completed within the last 6 months
- Current TB test (within the last 12 months)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Must be able to attend orientation at our Yuba City office
Nice To Haves
- Home Health experience preferred
- Experience with OASIS and WellSky (Kinnser) strongly preferred
- Med-Surg and wound care experience preferred
Responsibilities
- Obtain medical histories and assess patient conditions and clinical needs
- Conduct comprehensive nursing assessments including vital signs and mobility evaluations
- Educate patients and families on treatment plans, disease management, and home health services
- Deliver skilled nursing care while maintaining HIPAA compliance
- Complete accurate and timely clinical documentation and visit records
- Develop, implement, and revise individualized care plans as appropriate
- Coordinate care with LVNs/LPNs, Home Health Aides, therapists, and additional interdisciplinary team members
- Facilitate referrals for additional services including rehabilitation and medical social work
- Evaluate patient response to treatment and adjust care plans accordingly
- Manage assigned caseload while meeting 30-point weekly productivity expectations
