About The Position

The Registered Nurse (RN) provides care and services to a group of hospice patients by visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The RN contributes to the interdisciplinary team by providing expertise in psychosocial assessments and interventions and knowledge of community resources. The position may be in Case Management, Admissions, or a Resource role.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
• Ability to work collaboratively in a team environment.
• Ability to work independently on a day-to-day basis.
• Ability to communicate by phone, email, and in writing and to meet with a variety of healthcare professionals.
• Understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Responsibilities

• Provide care and services to a group of hospice patients.
• Visit patients in their residences, which may include private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team with expertise regarding psychosocial assessments, interventions, and community resources.
• Work in a Case Management, Admissions, or Resource role.
• Manage patient visits, documentation, meetings, and urgent needs by prioritizing and triaging concerns.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
