Per Diem Medicare Home Health Registered Nurse for Palm Bay and Melbourne
About The Position
The Registered Nurse (RN) will provide the highest quality of care to patients utilizing the Plan of Care to ensure patient's needs are met in relation to safety, well-being, and satisfaction. This is a part-time position covering the Palm Bay and Melbourne areas. The role involves providing safe and effective nursing care based on the nursing scope of practice and individualized Plan of Care, in conjunction with the Nurse Supervisor/Manager of Clinical Practice. Responsibilities may include medication administration, wound care, infusion, patient transfers and positioning, catheter care, and effective communication with the client, family, and Team Case Manager. The ideal candidate will demonstrate empathy, attention to detail, problem-solving skills, and a sense of urgency.
Requirements
- Current RN license, state of Florida
- Possess wound care and infusion skills
- Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
- 1 year of Home Health and Oasis experience required
- Requires a screening through the Care Provider Background Screening Clearinghouse.

- WellSky/Kinnser, Oasis & Home Health experience is a plus.
Responsibilities
- Provides safe and effective care for the client based on individualized Plan of Care.
- Provides prompt communication of changes in patient medical status/emergencies to Team resulting in effective management of cases.
- Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice.
- May include, but not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager
- Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
