About The Position

BAYADA is hiring Registered Nurses to provide home health care in the Upper Montgomery County area. The role involves making home visits, delivering care according to each patient’s designated care plan, monitoring and documenting patient conditions and services, and coordinating care. BAYADA offers flexible scheduling and one-to-one care, and will work with nurses to create a schedule that works for them and their family. Full-time, part-time, and per-diem opportunities are available.

Requirements

Hold a current Registered Nurse license in Pennsylvania.
Have a minimum of one year of recent, verifiable clinical medical/surgical experience.
Graduate from an accredited and approved nursing program, as indicated by a school transcript or diploma.
Be able to work independently and manage time effectively.
Have strong interpersonal skills.
Be able to travel to assigned cases.

Nice To Haves

Prior home care experience is strongly preferred but not required.
Prior experience with electronic medical records (EMR) is preferred.
Solid computer skills.

Responsibilities

Follow a designated care plan in accordance with patient needs.
Make home visits to clients in designated geographic territories.
Perform assigned duties, including medication administration, wound care, treatments, and procedures.
Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
Follow up on, execute, and properly document doctors’ orders.
Perform client assessments as necessary.
Provide case management and care coordination.
Accurately document observations, interventions, and evaluations related to client care and services using a touch pad tablet.
